It is the third installment in the Wonder Boy series and the last for arcades, featuring new protagonists: the warrior boy Leo (called Adam in some versions) and the young princess Purapril. It can be played single- or double-player, not alternating but cooperatively: player 1 controls Leo, player 2 controls Purapril. A re-release for the PC Engine was released in Japan in 1989.
A version for the Sega Genesis was also released in 1990, containing only nine levels instead of fourteen (the game had been scaled down due to the memory capacity of the cartridges in use at the time).
